Defining Post-COVID Symptoms (Post-Acute COVID, Long COVID, Persistent Post-COVID): An Integrative Classification

Abstract: The pandemic of the coronavirus disease 2019 (COVID-19) has provoked a second pandemic, the “long-haulers”, i.e., individuals presenting with post-COVID symptoms. We propose that to determine the presence of post-COVID symptoms, symptoms should appear after the diagnosis of SARS-CoV-2 infection; however, this situation has some problems due to the fact that not all people infected by SARS-CoV-2 receive such diagnosis. Based on relapsing/remitting nature of post-COVID symptoms, the following integrative classif… Show more

“…This review/meta-analysis considered original research including observational cohort or case-control studies where samples of COVID-19 survivors, either hospitalized or non-hospitalized, were followed for the presence of symptoms for more than two weeks after infection. Based on pre-existing data and timeframes [7] , we selected 30 days, 60 days, and ≥90 days after symptoms onset as pre-endpoints selected for the analysis. Editorials, opinion, and correspondence articles were excluded.…”

“…However, this review pooled prevalence rates without considering follow-up periods after symptoms and did not differentiate between hospitalized and non-hospitalized patients [6] . These two considerations are highly important to properly determine the presence of post-COVID-19 symptoms [7] .…”

“…Lastly, Fernandez-de-Las Penas et al considered also undiagnosed cases and proposed a time-based classification as follows: potentially infection-related symptoms (up to 4–5 weeks), acute post-COVID symptoms (from week 5 to week 12), long post-COVID symptoms (from week 12 to week 24), and persistent post-COVID symptoms (lasting more than 24 weeks). Intrinsic and extrinsic predisposing factors are also considered [ 23 ]. A consensus-based standardization of definition and classification of post-COVID syndrome is needed to provide a common denominator for diagnostic and therapeutic approaches, but also for research purposes.…”

“…All studies included in these meta-analyses had follow-up periods ranging from 3 to 12 weeks [ 6 , 8 , 9 ]. Our research group proposed an integrative model defining time reference points where persistent post-COVID-19 symptoms are those lasting longer than 24 weeks after the infection [ 10 ]. This multicenter study presents prevalence data and associated risk factors of post-COVID-19 cough one year after hospital discharge in a sample of subjects who had survived hospitalization for COVID-19.…”
