Degenerative type of placental chorioangioma requiring fetal blood transfusion

Abstract: We experienced a case with fetal hydrops, polyhydramnios, and a well‐defined oval anechoic lesion of approximately 9 cm in size, without blood flow at 26 weeks' gestation. As increased middle cerebral artery peak systolic velocity, the fetal hydrops was caused by a placental tumor such as a chorioangioma; however, the tumor was atypical. Fetal blood hemoglobin was 8.3 g/dl on percutaneous umbilical cord blood sampling. After erythrocytes transfusion to the fetus, the mother normally delivered at 38 weeks' gest… Show more

“…The neonate manifested cardiomegaly, microangiopathic hemolytic anemia, and thrombocytopenia, which resolved over time, and was attributed to diffuse placental chorangiomatosis [12] . Similarly, fetal anemia has been reported in association with placental chorioangioma [13 , 14] .…”

“…The capillary type is characterized by numerous capillary enlargements, the cellular type by dense parenchyma of immature cells with fewer angiogenesis, and the degenerative type by diverse degenerative changes, including fibrinoid necrosis and calcification. 16,22 The vascularization of chorioangiomas is a prognostic factor affecting pregnancy outcomes. While non-vascularized chorioangiomas are generally uncomplicated, vascularized tumors can lead to various complications 23 .…”

“…The mean Apgar scores at 1 and 5 min were low (5.96 ± 2.53; 7.5 ± 2.09), indicating poor prognosis and unfavorable neonatal outcomes. In fact, only 3 neonates (6.38%) had good clinical condition at birth [ 16 , 17 , 48 ]. In our case, the neonate had a favorable outcome; the Apgar score was higher than the mean value identified in other studies.…”
