Delayed Diagnosis of Neck Pain: Eagle Syndrome

Abstract: Eagle syndrome is a rare condition characterized by an abnormally elongated styloid process with or without abnormal direction and/or ossification of the styloid ligament. Clinically, it consists of throat and neck pain radiating to the ear. Here, we present the case of a 34-year-old female with the complaint of left-sided neck pain below the ear for the past year. The patient had tried different analgesics after seeing different doctors, but the pain did not resolve. After conducting radiological investigatio… Show more

“…Eagle syndrome (elongated styloid process) was initially described by Watt Weems Eagle who was an American otorhinolaryngologist. It is an abnormality of ossification/morphology of the styloid process [1][2][3][4]. The styloid process is a slender outgrowth at the base of the temporal bone, immediately posterior to the mastoid apex.…”

“…There is no association between age and Eagle syndrome. Although the majority of patients are middle-aged and elderly 12,19 , younger patients may suffer from this entity 13,17,26 . There seems to be a slight predominance in female patients 27 .…”

“…Treatment includes drug therapy (duloxetine, gabapentin, pregabalin, dexamethasone) and surgical excision of the styloid process 17,18,23 . Most authors prefer the surgical styloidectomy 12,13,22,26,28 . In some patients, partial styloidectomy is preferred to complete resection 20 .…”

“…In this variant, known as stylocarotid artery syndrome (SAS), the elongated SP compresses or protrudes into the ICA ( Figure 1 ), causing transient neurologic symptoms, including transient ischemic attacks (TIA) and stroke [ 2 , 3 ]. Therefore, identification of the cause and diagnosis of the underlying disease are often delayed [ 4 ]. Surgery (styloidectomy) is indicated in cases of refractory pain where other methods, including pharmacological treatment, have failed but it seems to be a necessary procedure in the diagnosis of pressure and/or dissection of the ICA.…”

“…ES was initially described in 1937 by W.W. Eagle, as facial, neck, and laryngeal pain associated with elongation of the ipsilateral SP, with or without calcification of the SL. The length of the SP ranges from 25 to 30 mm and is elongated in 4–7% of the population [ 4 ], usually asymptomatic (up to 30%), and symptomatic, as we mentioned aboved – in about 4% to 10%. Between the external and ICA lies the clinically important apex of the SP, as in our patients.…”
