Dental implant failure associated with a residual maxillary cyst

Abstract: A 53-year-old male underwent oral implantation including bone grafting to substitute tooth 12. Three months post-operatively, the implant migrated apically into a cyst cavity. The implant, cyst and bone graft were removed. This report regards the migration of an implant into a residual cyst; some anatomical, clinical and pathological considerations arise from this case

“…As Table 1 shows, there is still no consensus on the classification of IPL. The included reports (70 patients/76 implants) were preliminarily distributed in Penarrocha-Diago et al ’s classification [ 5 ]: 31 implants were inactive [ 8 , 12 , 13 , 17 , 18 , 20 , 21 , 26 , 27 , 29 , 30 , 33 , 36 ], and 46 implants were active, with acute non-suppurated(1) [ 13 ], acute suppurated (10) [ 6 , 7 , 14 , 16 , 19 , 25 , 28 , 31 , 32 ], subacute/suppurated-fistulized(7) [ 15 , 22 – 24 , 33 , 34 ], unclear stage(26) [ 35 ]. It should be noted that histopathological findings in some cases did not match Penarrocha-Diago et al’s classification to some extent.…”

“…In one such case the implant was placed in close proximity to the radiolucency because the region apical to the implant was taken as the anterior border of the sinus when actually it was a pathological osteolytic lesion. 16 It would be difficult to speculate regarding the cause of the cyst but it is worthy noting that this occurred around an integrated endosseous titanium implant and subsequently lead to bone loss and failure.…”

“… 5 The presence of an RC in an edentulous area prevents the functional rehabilitation of the patient due to the resulting bone defect. 6 All these factors justify the need for the surgical treatment of odontogenic cysts. In the present study, the lesion reached great proportions, which suggests a long course of evolution, despite the fact that the patient was asymptomatic.…”
