Dental implants and risk of bleeding in patients on oral anticoagulants: a systematic review and meta-analysis

Abstract: Background Dental implant placement is safe and predictable, yet optimal management of anticoagulated patients remains controversial. Whilst cessation of anticoagulation pre-operatively should decrease risks of bleeding, risk of thrombosis increases. We aim to define risk of bleeding in patients on oral anticoagulation who are undergoing dental implant placement, in order to establish best management. Methods This systematic review is registered wi… Show more

“…Kämmerer et al 170 have not stopped anticoagulant therapy. 171,172 Analogous results have been achieved in studies of patients on AT. 173 Furthermore, some authors also report a greater risk of thromboembolic events or myocardial infarction which may derive from the interruption of therapy with DAO or with antiplatelets, respectively.…”

“…Kämmerer et al 170 concluded that in patients with INR <4 treated with vitamin K antagonists dental surgery procedures can be performed safely without suspending therapy, implementing local hemostatic measures. Similarly recent reviews found a clinically insignificant risk of peri‐ and post‐operative bleeding in patients who have not stopped anticoagulant therapy 171,172 . Analogous results have been achieved in studies of patients on AT 173 .…”

“…Dawoud's systematic review focused on hemorrhagic complications in anticoagulated patients undergoing dental implantation. The review suggested that the continuation of anticoagulants in the peri-operative phase during dental implant surgery increases the risk of clinically insignificant peri-and postsurgical hemorrhage [22].…”
