Deprescription in elderly: A spotlight on pharmacoeconomic aspect

Abstract: Aging is a complex process and many factors in the elderly, especially multiple diseases and related unnecessary drug use, support a deprescription approach to this age group to save money and health cost. In this review, we have searched for studies related to the pharmacoeconomic aspect of this deprescription approach in the elderly. Few studies are available, but they are promising and effective in paving the way for prospective longitudinal studies to assess the role of deprescription in optimizing the dru… Show more

“…Due metabolic changes related to aging, different compounds may have its pharmacokinectic and biotransformation altered compared to younger subjects. Besides that, chronic diseases of the elderly may offer special risk together drug interactions and both may lead different patophysiological processes (Cadieux, 1989) Some drugs can cause adverse reactions related to cognitive impairment, such as benzodiazepines and anticholinergics (El Desoky, 2019). Cognitive impairment in the elderly can be partly associated with hypertension, and cognitive assessment tests can be used as an alternative in clinical observation (Williams et al, 2018).…”

“…Potansiyel İlaç etkileşimi sayısı arttıkça morbidite ve mortalite arttığından aşırı polifarmasi dediğimiz on adetten fazla ilaç kullanımı durumlarıyla karşı karşıya kalınmaktadır ve bu bir kısır döngüye dönüşmektedir. Bu sebeple hastaların tedavisinde yer alan tüm ilaçlar klinik karar destekleri kullanılarak etkileşim bakımından bütünüyle değerlendirilerek, kanda ilaç düzeyi izleme, ilacı değiştirme veya eksiltme gibi tedavi rejimlerine gidilmelidir 19,20 15,[21][22][23][24] . Çalışmamızın HYP veri tabanındaki hastalar üzerinde yapılması, sadece reçete edilen ilaçlar üzerinden değil hastaların kullandığı tüm ilaçların bakılmasıyla, ortaya çıkan ilaç etkileşimlerinin hastalara ulaşılarak kişiselleştirilmiş tedaviye yönelik adımlar atarak kliniğe yansımasıyla katkı sağlamıştır.…”
