Background: Experienced emotions can affect the outcome of, and adherence to a cardiac rehabilitation program, and patients coping with an illness. With more awareness of the expressed emotions, health professionals might be better able to understand the reactions of patients and to improve the support needed for coping. Living with an Implantable Cardioverter Defibrillator can lead to anxiety and depression. Focus on the intensity of the primary emotions might be a potential to prevent development of these psychological states.Objectives: The aim of this paper are 1) to describe the intensity of primary emotions in patients after implantation of an Implantable Cardioverter Defibrillator and 2) to compare them with both the intensity of primary emotions in patients with a recent Myocardial Infarction and with a healthy population. Method:The intensity of primary emotions in patients receiving an Implantable Cardioverter Defibrillator was measured by a survey using the Emotions and Health Scale. The survey was carried out as part of a randomized clinical trial the COPE-ICD trial (ID: NCT00569478) and is regarded as post-hoc explorative analysis. All data were collected from October 2007-November 2009. A total number of 154 patients were needed in the trial. The data collected were compared with secondary data from patients who had recently suffered a Myocardial Infarction and from a healthy population. SPSS version 18.0 was used to analyze the data using t-test, chisquare and F-test. Results:One hundred and ninety-six patients were invited to fill in the questionnaire. It was completed by 182, a response rate of 93 %. Patients with a newly implanted Implantable Cardioverter Defibrillator experienced an increased intensity of joy, sadness, disgust, anticipation, anger, fear and surprise. No difference was found regarding the intensity of the primary emotions when the group of patients was divided according to occupation or age. A significant increase in the negative emotions sadness, disgust, anger and surprise was observed in women and in patients who receive the Implantable Cardioverter Defibrillator as a secondary prevention. The combination of increased senses of fear and surprise is unique to patients receiving an Implantable Cardioverter Defibrillator. The intensity of the primary emotions in patients with a newly implanted Cardioverter Defibrillator was comparable to the intensity in patients with a myocardial infarction, but the experience of anticipation and fear was markedly increased in patients with an Implantable Cardioverter Defibrillator. Conclusion:Women and patients who receive an Implantable Cardioverter Defibrillator as secondary prevention experience a significant increase in the negative emotions sadness, disgust, anger and surprise.

Arrhythmogenic right ventricular cardiomyopathy/dysplasia (ARVC/D) is an inherited heart muscle disease that occurs primarily in young and middle-age individuals. It is characterized by ventricular arrhythmias (VA), sudden death, and by heart failure occurring later in life (1-5). Ventricular electrical instability may occur at any time during the disease depending upon possible different pathophysiologic mechanisms including: a) Inflammation and apoptosis leading to ventricular fibrillation; or b) Fibro-fatty tissue repair leading to scar-related ventricular tachycardia (VT). Heart failure may occur later in life secondary to slow, progressive loss of right and left ventricular myocardium (1-8). The role of pharmacological therapy in controlling VA and preventing sudden death has been evaluated in single-center studies (9), and the results of catheter ablation have been described in small series of patients (10-15). The efficacy and safety of implantable cardioverter/ defibrillator (ICD) have also been reported in small, single-center studies (16)(17)(18)(19) and recently in larger single and multicenter studies (20)(21)(22)(23)(24)(25)(26)(27). The main questions regarding the risk stratification and the therapeutic strategy in ARVC/D are: 1) differential diagnosis with idiopathic VT (right ventricular outflow tract VT) in an apparently normal heart. The prognosis of this latter condition is usually excellent with rare cases of sudden cardiac death; 2) prognostic and therapeutic significance of noninvasive and invasive investigations including electrophysiologic study (EPS); 3) efficacy of antiarrhythmic drugs (AAD) to prevent VT and sudden cardiac death and the adverse effects of these drugs in this population; 4) indications and results of catheter ablation; 5) identification of patients at high risk of sudden cardiac death who need ICD implantation as well as the complications of ICD in a diseased right ventricular myocardium. It is important to recognize that ARVC/D is a progressive disease and risk factors may change during follow-up requiring periodic revaluation of risk as well as of therapy.With the identification of family members who carry a disease causing gene, the therapeutic dilemma has broadened to include risk stratification in genotype positive family members who may have occasional ventricular ectopy or have no clinical evidence of the disease.Publisher's Disclaimer: This is a PDF file of an unedited manuscript that has been accepted for publication. The success of the implantable cardioverter defi brillator (ICD) in several large -scale randomized trials has established the ICD as the treatment of choice in detecting and treating ventricular arrhythmias (AVID Investigators 1997 ;Moss et al. 2002 ;Bardy et al. 2005 ). Patientreported outcomes encompassing psychosocial and quality of life (QOL) outcomes have generally been positive and desirable. Nonetheless, living successfully with the risk of spontaneous, potentially life -threatening arrhythmias and the resulting, life -saving high -energy shocks presents challenges to a sizable minority of patients and their families. ICDs save lives in at -risk patients, but the ongoing need for comprehensive, biopsychosocial care plans addressing the range of issues for both patients and families has been a challenge in electrophysiology clinics. Researchers have been stimulated by this modern -day scenario to describe, predict, and treat the potential for general psychological distress (i.e., anxiety and depression) and avoidance behaviors in ICD patients. More recent research has added the perspective of ICD family members, such as partners/spouses/ signifi cant others, to better understand the collective adjustment challenges. Preliminary fi ndings indicate that this may be particularly needed as ICD spouses experience signifi cant distress as well. The purpose of this chapter is to review the overall psychosocial and QOL impact of ICDs on patients and their partners. Clinical management strategies and approaches will also be discussed.

Abstract: Our study indicates presence of depression and anxiety in nearly half of the patients with ICD. Consultation with psychiatry should be a part of the treatment for patients with ICD, especially for those who constitute high-risk groups.
