Depressive symptoms, chronic medical conditions and functional status: a comparison of urban and rural elders in Taiwan

Abstract: The present findings confirm urban-rural differences in the reporting of associations between depressive symptoms, chronic medical conditions, and functional disability. For developing prevention programs on geriatric depression, it is critical to call more attention to chronic medical conditions, functional status, and social support of urban and rural elderly.

“…It is clearly demonstrated in the present study that rural and urban elderly differ significantly in the prevalence of depression. It also conforms to the views expressed in some literature that place of residence is an important influencing factor for emergence of the state of depression among the elders [7,11,12].…”

“…In addition to the above-mentioned cross-sectional studies which have indicated a relationship between depression and functional capacity among older adults (Nourhashemi et al, 2001;Chiu et al, 2005;Friedman et al, 2005;Santos et al, 2008), longitudinal studies have recently reported on the relationship between depression and a decline in functional capacity (i.e., functional decline) (Bruce et al, 1994;Penninx et al, 1999;Wang et al, 2002;Lenze et al, 2005;Anstey et al, 2007). However, these longitudinal studies had a relatively short follow-up period of less than 10 years.…”

“…It is not only thought to sharply decrease quality of life but is also thought to be a strong risk factor for suicide among older adults (Mann et al, 2005). In addition, it has been reported that depression is related to various physical impediments, such as chronic diseases (including diabetes mellitus (Talbot and Nouwen, 2000), heart disease (Barefoot and Schroll, 1996;Glassman and Shapiro, 1998), hypertension (Jonas and Lando, 2000), stroke (Jonas and Mussolino, 2000)), mortality (Takeshita et al, 2002;Ahto et al, 2007), physical fitness (Penninx et al, 1998;Penninx et al, 2000;Yanagita et al, 2006), and functional capacity (Nourhashemi et al, 2001;Chiu et al, 2005;Friedman et al, 2005;Santos et al, 2008). Therefore, it is thought that the early detection and treatment of depression among older adults is crucial.…”
