Derivation and validation of a 90-day unplanned hospital readmission score in older patients discharged form a geriatric ward

Abstract

Key summary points Aim To derive and validate a 90-day unplanned hospital readmission (UHR) score based on information readily available to non-hospital based care providers. Findings Independent risk factors for 90-day UHR were: use of mobility aids, presence of dementia syndrome, history of recent hospitalisation, and discharge to domiciliary home. In the development cohort and in the validation cohort, the 90-day UHR rate increased significantly across risk groups. Message The findings should enable targeted multidisciplinary interventions in order to limit UHR.
