Dermoscopy Findings of Hidroacanthoma Simplex

Abstract: Hidroacanthoma simplex (HAS), also known as intraepidermal eccrine poroma, is a rare eccrine adnexal tumor that tends to be misdiagnosed as other types of benign skin tumor, including clonal seborrheic keratosis. Notably, HAS is sometimes misdiagnosed and treated by cryosurgery as seborrheic keratosis, which could trigger the later development of porocarcinoma. Therefore, accurate diagnosis of HAS is indispensable for dermatologists to avoid the development of malignant tumors by an unsuitable treatment. In th… Show more

“…Like in flat SKs, also our case of PHS (Figure 3C) showed fingerprint-like structures around the hair follicles and comedo-like openings; however, the patient did not present any other similar lesions on the skin; therefore, an incisional biopsy was performed. In the literature, these dermoscopic and histopathological findings were reported when incisional biopsies were performed in the thinner part of the lesions [14,15].…”

“…The white-blue color was not present at the periphery, but rather in the center of the lesion and, although polymorphous vessels were present, there were no crystalline structures and multiple erosions, as in eccrine poroma [ 10 ]. In our case, the lesion did not show whitish globular structures surrounded by homogenous, pigmented lines as in hidroacanthoma simplex [ 11 ]. Multiple red lagoons intersected by white lines were absent, thus also differentiating it from pyogenic granuloma [ 12 ].…”

“…In the present case, HS dermoscopy revealed characteristic features, namely whitish globular structures surrounded by homogenous, pigmented lines, and globular structures covered with gatherings of the thick, oval keratotic materials, referred to as "muskmelon appearance". 10 In a recent report, dermoscopy of an eccrine porocarcinoma (EPC) arising from a HS revealed vessels in a conspicuous and irregular shape. 11 The differences on dermoscopy could be a helpful clue for the tumor grade.…”
