Descemet Membrane Endothelial Keratoplasty Donor Preparation

Abstract: With appropriate techniques, DMEK donor preparation can be highly successful, even for a novice surgeon. When a donor develops multiple HSTs, we recommend not using the mate for DMEK because of a higher risk of encountering a preparation difficulty.

“…In our series, dissection could be completed within 3 min. Other studies using different approaches described longer preparation times (mean 10.1 ± 3.3 min) and a variable tearing rate of 2 to 13 % [11][12][13]. In our series, the graft failure rate was only 1.16 %.…”

“…8 In brief, a modified submerged cornea using a background-away technique was used to peel the endothelium and Descemet membrane from a donor cornea either the day of surgery or 1 to 2 days in advance. 9,10 Patients received topical anesthesia and monitored intravenous sedation. Before an inferior peripheral iridotomy was made, dysfunctional endothelium and Descemet membrane were stripped from an area of the recipient cornea that approximately matched the planned graft diameter.…”

“…DMEK is more technically difficult than DSEK, and it has a steeper learning curve. [8][9][10] The success rate has increased drastically with the improvement of the surgical technique since the first report of DMEK in 2006. 11 Many cornea surgeons have not started performing DMEK because of the challenge of donor preparation.…”
