Descemet stripping automated endothelial keratoplasty with the EndoGlide Ultrathin graft insertion device

Abstract: Endothelial keratoplasty has largely replaced penetrating keratoplasty as the preferred technique to selectively replace diseased corneal endothelium. Descemet's stripping automated endothelial keratoplasty (DSAEK) is the most common type of endothelial keratoplasty performed worldwide. One of the challenges in DSAEK is the insertion of the donor lenticule into the eye using a method so as to minimize endothelial cell loss. Various techniques have been suggested such as forceps insertion and graft insertion de… Show more

“…[10][11][12] In the first year following Descemet's membrane endothelial keratoplasty, Descemet's stripping automated endothelial keratoplasty, and penetrating keratoplasty, endothelial cell losses have been reported as 35%, 11 14.9%, 12 and 16.5%, 10 respectively. By the third postoperative year, endothelial cell losses will have increased to approximately 47%, 11 29.7%, 13,14 and 55%, 10 respectively. The progression of endothelial cell loss may eventually lead to graft failure and repeat graft surgeries in some patients.…”

“…Graft rejection and graft failure are less frequent in newer methods of DSAEK such as ultra-thin DSAEK or in DSAEK using donor insertion device (15)(16)(17). The results of this study can be used to compare different transplantation techniques, including traditional PK, and more importantly, newer endothelial keratoplasty methods such as DMEK and DMET.…”

“…Or they develop new technologies to solve problems that they have in the clinic. 3,4 Finally, the current research environment has required clinician scientists to develop the leadership skills to manage the complex healthcare system of the future. When I started out studying the genetics of human disease, we were carrying out what we thought were large studies involving a few hundred patients.…”
