Investigations in preventive and occupational medicine are often based on the acquisition of data in the customer's daily routine. This requires convenient measurement solutions including physiological, psychological, physical, and sometimes emotional parameters. In this paper, the introduction of a decentralized multi-sensor-fusion approach for a preventive health-management system is described. The aim is the provision of a flexible mobile data-collection platform, which can be used in many different health-care related applications. Different heterogeneous data sources can be integrated and measured data are prepared and transferred to a superordinated data-science-oriented cloud-solution. The presented novel approach focuses on the integration and fusion of different mobile data sources on a mobile data collection system (mDCS). This includes directly coupled wireless sensor devices, indirectly coupled devices offering the datasets via vendor-specific cloud solutions (as e.g., Fitbit, San Francisco, USA and Nokia, Espoo, Finland) and questionnaires to acquire subjective and objective parameters. The mDCS functions as a user-specific interface adapter and data concentrator decentralized from a data-science-oriented processing cloud. A low-level data fusion in the mDCS includes the synchronization of the data sources, the individual selection of required data sets and the execution of pre-processing procedures. Thus, the mDCS increases the availability of the processing cloud and in consequence also of the higher level data-fusion procedures. The developed system can be easily adapted to changing health-care applications by using different sensor combinations. The complex processing for data analysis can be supported and intervention measures can be provided.

O Registo de Saúde Eletrónico (RSE) é o resultado da mudança do atual sistema de saúde, influenciado pela inovação, integração e partilha de dados clínicos. O RSE deve incluir sumário clínico dos pacientes, prescrições eletrónicas, registo eletrónico de dispensa de medicação, resultados laboratoriais, imagens e/ou relatórios médicos e notas de alta hospitalar. A prescrição médica e a administração de terapêutica são dos erros evitáveis mais comuns em Saúde, a sexta maior causa de morte nos EUA e uma prioridade para a Organização Mundial de Saúde desde que lançou a iniciativa “Medication Without Harm”. Os objetivos deste trabalho são: i) apresentar um modelo digital de reconciliação terapêutica; ii) propor uma solução interoperável e legalmente conforme; iii) integrar a tecnologia Blockchain e Inteligência Artificial no modelo apresentado. Através de uma revisão bibliográfica descritiva e exploratória, propôs-se a utilização dos “standards” HL7 FHIR e SNOMED-CT, modelo “client-CA” para a Blockchain e modelos de “machine learning” e “natural language processing” para a Inteligência Artificial. É complexo garantir a transmissão de dados clínicos de forma interoperável e segura, mas teoricamente exequível. As vantagens ao nível da reconciliação terapêutica serão mensuráveis com a contínua diminuição da morbimortalidade associada ao erro terapêutico.

The need for cloud services has been raised globally to provide a platform for healthcare providers to efficiently manage their citizens’ health records and thus provide treatment remotely. In Iraq, the healthcare records of public hospitals are increasing progressively with poor digital management. While recent works indicate cloud computing as a platform for all sectors globally, a lack of empirical evidence demands a comprehensive investigation to identify the significant factors that influence the utilization of cloud health computing. Here we provide a cost-effective, modular, and computationally efficient model of utilizing cloud computing based on the organization theory and the theory of reasoned action perspectives. A total of 105 key informant data were further analyzed. The partial least square structural equation modeling was used for data analysis to explore the effect of organizational structure variables on healthcare information technicians’ behaviors to utilize cloud services. Empirical results revealed that Internet networks, software modularity, hardware modularity, and training availability significantly influence information technicians’ behavioral control and confirmation. Furthermore, these factors positively impacted their utilization of cloud systems, while behavioral control had no significant effect. The importance-performance map analysis further confirms that these factors exhibit high importance in shaping user utilization. Our findings can provide a comprehensive and unified guide to policymakers in the healthcare industry by focusing on the significant factors in organizational and behavioral contexts to engage health information technicians in the development and implementation phases.
