Design of the P acemaker RE mote F ollow-up E valuation and R eview (PREFER) trial to assess the clinical value of the remote pacemaker interrogation in the management of pacemaker patients

Abstract: Background: Although pacemakers are primarily used for the treatment of bradycardia, diagnostic data available in current pacemakers allow them to be also used as sophisticated, continuous monitoring devices. Easy access to these stored data may assist clinicians in making diagnostic and therapeutic decisions sooner, thus avoiding potential long-term sequelae due to untreated clinical disorders. Internet-based remote device interrogation systems provide clinicians with frequent and complete access to stored da… Show more

“…These events, as defined in Chen et al (10) and listed in Table 1 Pacemaker Remote Management of problems with the pacing system may also be noted, including a significant increase in stimulation threshold, changes in lead impedance, and indicator of battery depletion. Each CAE requires, at minimum, physician diagnostic action and could require the addition of a medication or surgical revision of the pacing system.…”

“…Before the time is ripe to fully incorporate this new technology in daily cardiology, a period of learning and testing is needed. Several prospective trials have already been launched to assess the total number and types of clinically actionable events and possible benefits, such as the PREFER study 11 and the TRUST study. 12 A Dutch nationwide prospective comparative study to examine several aspects (table 2) of remote monitoring of implanted cardiac devices can bring out a tailored and sustainable remote device monitoring for the next generation of device recipients.…”

“…Because insurance companies dictate face-to-face visits for the follow-up of active devices after implantation, evidencebased indications and rules should be formulated in the short term to convince them that remote monitoring improves the quality of follow-up and saves time and budget. 11 These outcomes serve to defend the opinion that all parties involved in remote monitoring deserve reimbursement according their efforts.…”
