Design optimization of high tibial osteotomy plates using finite element analysis for improved biomechanical effect

Abstract: Background High tibial osteotomy (HTO) is a common treatment for moderate osteoarthritis of the medial compartment in the knee joint by the translation of the force center toward the lateral compartment. However, the stability of a short plate such as Puddu used in this procedure was not as effective as other long plates such as Tomofix. No previous studies have used a rigorous and systematic design optimization method to determine the optimal shape of short HTO plate. Therefore, the purpose of th… Show more

“…Third, we evaluated patients with OA after the removal of metal components. Finite element analysis has been used to demonstrate the stress shielding effect of the HTO plate on the bone around the plate 22 ; however, the mechanisms underlying this effect on the articular surface are unclear. Moreover, regardless of the stress shielding effect of the HTO plate, this study reveals that the distribution of subchondral bone density differs significantly before and 1.5 years after HTO.…”

“…In addition, the 3D models were simplified with the use of pins instead of screws. This modification has also been used in past OWHTO studies with FEA [ 25 ]. Third, the osteotomy was not biplanar but monoplanar.…”

“…The elastic modulus of bone voxels were ranged from 5000 to 17,000 MPa, for cancellous and cortical bone, respectively, according to their HU numbers, with a constant Poisson’s ratio of 0.33. 16,22,23 The plate and screws were assumed to be made from Ti-6Al-4V alloy with modulus of elastic and Poisson’s ratio of 110,000 MPa and 0.37, respectively. 23,25…”

“…Similarly, for modeling the engagement of the bone and screw shafts, surface-to-surface contact conditions with a friction coefficient of 0.4 were considered. 16,25 Finally, for locking screws, the interface between screw heads and plate was bonded using tie contact condition, assuming a perfect lock between the screw heads and the threaded holes of the plate. 23…”
