Desmoplastic Melanoma: Is There a Role for Sentinel Lymph Node Biopsy?

Abstract: Background The utility of sentinel lymph node biopsy (SLNB) for desmoplastic melanoma (DM) is debated. We describe a large single-institution experience with SLNB for DM to determine clinicopathologic factors predictive of SLN metastasis. Methods Retrospective review identified 205 patients with DM who underwent SLNB from 1992 to 2010. Clinicopathologic characteristics were correlated with SLN status and outcome. Results Median age was 66 years, and 69 % of patients were male. Median Breslow thickness was … Show more

“…One study specifically examining thick melanomas and SLNB was performed by Gershenwald et al and reported a successful SLNB rate of 96%. The SLN positivity rate was 39% . As in intermediate and thin melanomas, patients with positive SLN are offered CLND of the affected lymph node basin, which is consistent with NCCN guidelines.…”

“…The authors concluded that these rates are high enough to justify the use of SLNB in both variants of DM. 84 As such, at MCC we routinely offer SLNB to patients with DM. However, other major cancer centers do not perform SLNB in patients with DM.…”

“…Because desmo-plastic melanoma spreads to the lymph nodes in only a minority of cases (12% in the current series and 14% in a previous analysis performed by our group among patients with desmoplastic melanoma who underwent sentinel lymph node biopsy 19 ), regional RT was rarely used. As a result, the role of local plus regional lymph node RT could not be adequately addressed.…”

“…The SLN procedure can be indicated, even though it is a rare situation and the bibliography is not so large (one investigator found positive SLNs in 9 % of patients) [46][47][48][49]. The incidence of lymph node metastases in patients with mixed-type desmoplastic melanoma appears to be the same as in those with non-desmoplastic melanoma [48].…”
