Deszendierende nekrotisierende Mediastinitis

Abstract: Rapid diagnosis, anatomical knowledge, understanding of the progression of infections as well as critical care, antimicrobial treatment and multidisciplinary radical surgical therapy are paramount for successful treatment of DNM. We favor anterolateral thoracotomy as the standard open transthoracic approach to the mediastinum. Placement of cervico-mediastino-thoracic irrigation drains can help to limit DNM.

“…Antibiotic-loaded bone cement (ALBC), such as PMMA, is widely used by orthopedic surgeons to help secure arthroplasty implants, to fill bone voids, and to treat vertebral compression fractures ( Figure 5) [57,58] . ALBC has been in use since first being developed in 1970 as a potential method for in situ drug release [59] . Despite its widespread use, the antimicrobial efficacy of ALBC is debated [60,61] .…”

“…Polymethylmethacrylate (PMMA) bone cement has a long history of use as an antibiotic carrier, with Buchholz and Engelbrecht first publishing on antibiotics mixed with Palacos cement in 1970 [ 48 ]. Structural cement spacers that bear load are an important part of staged revision for PJI.…”
