Objectives The purpose of this study was to use speckle tracking analysis to evaluate the size, shape, and function of the atrial chambers in normal fetuses and develop a z‐score calculator that can be used in future studies in fetuses at risk for cardiovascular disease. Methods The control group consisted of 200 normal fetuses examined between 20 and 40 weeks of gestation in which speckle tracking analysis of right (RA) and left (LA) atrial chambers was performed. The atrial end‐diastolic and end‐systolic endocardial borders for each chamber were identified from which measurements of atrial length, width, area, and volume were computed. Equations were derived using fractional polynomial regression analysis to compute z‐score equations. Results The LA end‐diastolic volume, RA and LA end‐diastolic area, length, base width, and mid‐chamber widths increased with gestational age and fetal size. Left atrial emptying and ejection volumes increased with gestational age and fetal size. The fractional area change was significantly less for the RA than the LA. The LA base and mid‐chamber fractional shortening were significantly greater than the RA. There was a significant difference between the RA and LA global contractile strain. Conclusion Mean and standard deviation equations for each of the measurements described in this study were computed to create a z‐score calculator that can be utilized in the clinical environment when evaluating fetuses with suspected atrial pathology that could alter the size, shape, and function of the atrial chambers.

ObjectiveThis study aimed to compare cardiac morphological and functional changes in fetuses of patients with diet‐regulated gestational diabetes mellitus (GDM‐A1), insulin‐regulated GDM (GDM‐A2), and a control group.MethodA prospective cohort study included pregnant women aged 18–40 years with singleton pregnancies. Fetal biometric, cardiac morphological, and functional measurements were recorded using Z‐scores at 34–37 weeks of gestation.ResultsThe study included 87 patients. Both right and left ventricular wall thicknesses were significantly different between the three groups (p < 0.001 and p < 0.001, respectively). Z‐scores of the mitral valve, left and right EDD were significantly lower in GDM‐A1 and GDM‐A2 groups compared to the control group (p < 0.001, p < 0.001, p = 0.002, respectively). Right and left ventricular areas were decreased only in GDM‐A2 group compared to the control group (p = 0.003 and p = 0.001, respectively). MPI and IVRT values were also significantly higher in the same groups (p = 0.016, p < 0.001, respectively).ConclusionGestational diabetes increased IVS and ventricular wall thicknesses in both right and left ventricles, irrespective of whether it was controlled by diet or insulin. Cardiac functional changes were observed in the GDM‐A2 group.

Hypoplastic left heart syndrome (HLHS) is characterized by underdevelopment of the left-sided heart structures. The prenatal diagnosis of this congenital heart disease is crucial because a newborn with undiagnosed HLHS often presents with clinical signs of low cardiac output once the ductus arteriosus begins to close. With that in mind, the aim of this article was to perform a non-systematic review focusing on the key ultrasound features that can be used in the prenatal diagnosis of HLHS. Severe forms of HLHS are characterized by a markedly abnormal four-chamber view of the fetal heart (small left atrium, hypoplastic left ventricle, or abnormal mitral valve). The left ventricular outflow tract view allows the degree of hypoplasia in the tract to be evaluated and the diameter of the ascending aorta to be measured. The Z-scores are intended to aid in the diagnosis and follow-up of HLHS. In mild forms of HLHS, a right ventricle/left ventricle length ratio > 1.28 was the strongest predictor of a univentricular outcome.
