Detecting High Hyperopia: The Plus Lens Test and the Spot Vision Screener

Abstract: In this preliminary study, the Plus Lens test and the Spot Vision Screener demonstrated moderate sensitivity with good specificity in detecting high hyperopia. [J Pediatr Ophthalmol Strabismus. 2017;54(3):163-167.].

“…Feldman et al . evaluated the plus lens test (+2.50D) in 109 children aged 2–12 years, reported a sensitivity and specificity of 38.1% and 89.7% for detecting hyperopia of >+2.50D, when the majority of letters could be read on the 6/9 line . Their results support the current findings of poor performance of the plus lens test for detecting hyperopia, in this case with a different plus lens power and a different definition of hyperopia, but with both studies demonstrating low test sensitivity.…”

“…A limitation of Feldman et al . 's study, was that participants were recruited from a paediatric ophthalmology clinic, and may be existing spectacle wearers; furthermore, it was not clear whether the plus lens test was undertaken over the spectacle correction. Children with corrected hyperopia may elicit a different response to the plus lens test when unaided (specifically, they will be more able to relax their accommodation to see through the plus lens) compared with uncorrected hyperopic children (who may be constantly over‐accommodating).…”

“…Ersetzt durch die heute übliche CyclopentolatSkiaskopie und in ausgesuchten Fällen den Photoscreener [1,2] hat die Skiaskopie viel an Schrecken verloren. Klare Richtlinien zur Brillenverordnung [1], eine empfohlene Brillenadaptationszeit und Okklusionsstrategie sowie Möglichkeiten zur Verbesserung der Compliance erleichtern die Behandlung der Amblyopie.…”
