“…The database search yielded 475 studies and, after removal of 134 duplicates, 341 studies were screened by reading through the title and abstract. From these studies, 120 were considered eligible for full text review, from which four met our inclusion criteria. Eighty studies were excluded because they did not provide data on ESBL‐E GI colonization, 12 studies did not provide the total number of screened patients, nine studies were reviews, seven studies did not include SOT patients, three studies had mixed data on infection and colonization, two studies were published in non‐English language, two studies contained overlapping data with the included articles, and one study presented a case series.…”

“…The observed genetic diversity in the fecal isolates has potential implications in an outbreak setting, where attempts to track the spread of individual clones have been customary [6,31,37]. It is relevant to question if such tracking is a reasonable approach, based on the observed diversity among ESBL producing fecal E .…”

“…The paper by O'Connell et al 1 in this issue is a reminder of both the ubiquity of extended-spectrum beta-lactamaseproducing Enterobacteriaceae (ESBL-E), and the lack of a consistent infection prevention and control approach to the threat posed by these bacteria. Many clinicians have become used to seeing patients with ESBL-E, but it is often unclear how to manage these patients from an infection prevention and control perspective, and possible actions may be limited by a lack of resources.…”

“…Many clinicians have become used to seeing patients with ESBL-E, but it is often unclear how to manage these patients from an infection prevention and control perspective, and possible actions may be limited by a lack of resources. In particular, there is usually limited availability of single rooms for isolation, and it is interesting that O'Connell et al 1 found that only 25% of survey participants in Ireland consistently isolated patients with ESBL-E.…”
