Detection of Atrial Fibrillation During Sinus Tachycardia Induced by Exercise in Patients with Implantable Atrial Defibrillators

Abstract: Accurate detection of atrial fibrillation (AF) is essential for appropriate operation of an implantable atrial defibrillator (IAD). However, during episodes of sinus tachycardia, distinction between AF and sinus rhythm (SR) using the "quiet interval" and "baseline crossing" analysis in the detection algorithm of the IAD may be difficult. The efficacy of this AF detection algorithm was tested in five patients implanted with an IAD (METRIX, Model 3000 or 3020, InControl Inc.) during treadmill exercise testing. T… Show more

“…Since atrial antitachycardia pacing and improved techniques for atrial cardioversion are evolving, it is of great clinical and scientific interest to gain information on the natural history of atrial tachyarrhythmias not only in patients with concomitant malignant ventricular tachyarrhythmias. To date, studies have been performed that focus primarily on patients with paroxysmal AF and only mild heart disease, as the Metrix study using a stand‐alone atrial defibrillator 13–18 . Furthermore, there is restricted data on the incidence of atrial tachyarrhythmias in patients with a single chamber ICD.…”

“…Furthermore, the data log contains the exact time of onset of the arrhythmia and its duration. There have been several reports on the incidence of atrial tachyarrhythmias in patients with a history of paroxysmal atrial fibrillation (AF) based on prospective studies with the implantable atrial defibrillator (Metrix, Guidant Inc., St. Paul, MN, USA) 13–18 . This device offers atrial electrograms and episode information on a restricted number of episodes.…”
