Detection of Extended-Spectrum β-Lactamases by Using Boronic Acid as an AmpC β-Lactamase Inhibitor in Clinical Isolates of Klebsiella spp. and Escherichia coli

Abstract: We evaluated highly sensitive methods using boronic acid (BA) to detect extended-spectrum ␤-lactamase (ESBL) production. A total of 182 clinical isolates of Klebsiella spp. (n ‫؍‬ 118) and Escherichia coli (n ‫؍‬ 64) were analyzed: 62 harbored only ESBLs, 80 harbored both ESBLs and plasmid-mediated AmpC ␤-lactamases (pAmpCs), and 40 harbored only pAmpCs. The CLSI confirmatory test detected all isolates that produce only ESBLs but detected 85% of isolates that produce both enzymes. When a >5-mm increase in the … Show more

“…Addition of 3-aminophenyl boronic acid on antibiotic-containing disks (cefotaxime/ceftazidime with or without clavulanate) has been reported to potentiate the sensitivity and specificity of the combination disk method when this method is applied to organisms with chromosomal [74] or plasmid-mediated [75] AmpC production.…”

“…Hodge and imipenem and EDTA-sodium mercaptoacetic acid double-disc synergy (IEDDS) tests were performed on agar plates as described previously (Lee et al, 2001(Lee et al, , 2010a. The MICs of imipenem and meropenem were determined using Etest strips (AB Biodisk) on Mueller-Hinton agar plates with or without a fixed concentration of clavulanic acid (CA; 4 mg ml 21 ; Sigma-Aldrich) as an inhibitor of ESBLs, aminophenylboronic acid (APB; 300 mg ml 21 ; Sigma-Aldrich) as an inhibitor of AmpC b-lactamase, or Phe-Arg-b-naphthylamide (PAbN; 20 mg ml 21 ; SigmaAldrich) as an efflux pump inhibitor (Song et al, 2007).…”

“…The MBL Etest (AB Biodisk, Solna, Sweden) and the combined-disk test with imipenem and EDTA (10) were used to screen for the production of class B carbapenemases. ESBL production was tested using the CLSI confirmatory test and a modification, which uses clavulanate in combination with boronic acid (26). Isolates were phenotypically tested for the presence of AmpC ␤-lactamase by determining cefoxitin and imipenem MICs in agar with and without 200 g/ml cloxacillin (23) and by using the AmpC detection Etest strips (AB Biodisk), which contain cefotetan with or without cloxacillin.…”
