Detection of hepatocellular carcinomas with near-infrared fluorescence imaging using indocyanine green: its usefulness and limitation

Abstract: Indocyanine green fluorography is useful to detect HCCs; however, attention should be paid to the fact that HCCs may be occasionally overlooked by this imaging method and that lesions detected by this method are not always neoplastic lesions.

“…For instance, ICG can clearly identify hepatocellular carcinoma lesions and colorectal cancer liver metastases during surgery. 147,148 Presently, intraoperative evaluation of tumor margins depends on palpation and visual inspection. NIRF probes with cancertargeting ability can provide instrumental information in demarcating tumors.…”

“…Intravenously administered ICG has also been used in NIR endoscopy systems to successfully differentiate superficial gastric tumors from invasive cancers (13). ICG has been shown to localize to both hepatocellular carcinoma and intrahepatic metastases 24-72 hours after injection with high target-to-background ratios (2,3,6). The mechanism by which ICG localizes to intrahepatic metastases has recently begun to be elucidated.…”

“…The intraoperative imaging of ICG for the detection of intrahepatic malignancy has been demonstrated (2,3,6). Indeed, OMI may add value to standard visual inspection and intraoperative ultrasonography (US) for the detection of superficial hepatic lesions during surgical resection (7).…”

“…PDT requires a chemical agent that is called as photosensitizer and activation of the agent by light of a specific wavelength to produce oxygendependent cytotoxic reaction [1][2][3] Indocyanine green (ICG), a photosensitizer with a molecular weight of 775 Da, has been used as a diagnostic agent to determine cardiac output, hepatic function and blood flow [4,5]. ICG has low toxicity and has been approved by Food and Drug Administration (FDA) [6].…”
