Detection of prostate cancer using prostate imaging reporting and data system score and prostate-specific antigen density in biopsy-naive and prior biopsy-negative patients

Abstract: Background Few studies report on indications for prostate biopsy using Prostate Imaging–Reporting and Data System (PI-RADS) score and prostate-specific antigen density (PSAD). No study to date has included biopsy-naïve and prior biopsy-negative patients. Therefore, we evaluated the predictive values of the PI-RADS, version 2 (v2) score combined with PSAD to decrease unnecessary biopsies in biopsy-naïve and prior biopsy-negative patients. Materials and methods A total of… Show more

“…In addition, compared to the PI-RADS v2.1 score, the cs-PCa detection rate was the highest (66.7%) for the combination of PI-RADS v2.1 score > 3 and PSAD ≥ 0.30 ng/mL/mL. Our results were substantially consistent with the previous PI-RADS v2.0 study by Ryoo et al [ 31 ]. They concluded that patients with PI-RADS v2 score ≤ 2 may avoid an unnecessary biopsy regardless of PSAD; patients with PI-RADS v2 score 3 may avoid an unnecessary biopsy through PSAD results.…”

“…The authors stated that the inclusion of PSAD <0.1 ng/ml/ml in the biopsy strategy for patients with no biopsy history and with suspicious mpMRI findings would result in a 43% reduction in prostate biopsies. In another study, Ryoo et al emphasized that prostate biopsy can be avoided in case of patients with PI-RADS-3 lesion, PSAD <0.15 ng/mL/mL and with no biopsy history (19). In our study, we did not find any statistically significant finding that PSAD increased PCa predictivity alone or in combination with other parameters in patients with PI-RADS-3 lesions.…”

“…MRI fusion biopsies have become increasingly common in the other countries such as the US. However, in Korea, such biopsies have only recently become reimbursed [40]. Our data was collected before MRI fusion techniques were commonly used.…”
