Determinants of quality of life in Alzheimer's disease: perspective of patients, informal caregivers, and professional caregivers

Abstract: Our findings suggest that depression is the main variable related to patients' QoL and that more careful management of neuropsychiatric disorders is necessary. Both proxies' ratings are not equivalent to patients' reports in terms of predictors but they are complementary. Thus, a thorough QoL assessment should consider separately the perspective of the different informants.

“…The impact of depression in this regard has been widely reported 7,41 and suggests the existence of differences between perceptions of psychological wellbeing that could be due to caregiver-related variables (personal beliefs or expectations). 43 Thus, it is acceptable to assume that there is an association between caregivers' perceptions of PwD QoL and PwD well-being in mild dementia. It seems that changes in caregiver scores are relatively dependent on the effect of change in other clinical parameters.…”

“…Although depression is the neuropsychiatric symptom most widely reported to be associated with poorer QoL, this association with impaired QoL has also been found for the total NPI score [5,11,17], as well as for other specific neuropsychiatric symptoms, including apathy [39], anxiety [2,4], agitation [40], and psychotic symptoms assessed by the NPI (Delusions, Hallucinations) [5].…”

“…Furthermore, the results obtained have been similar despite the variety of instruments used to assess depression: Cornell Scale for Depression in Dementia (CSDD) [2,4], Center for Epidemiologic Studies-Depression Scale (CES-D) [6], Hospital Anxiety and Depression Scale (HADS) [8], NPI-Depression [5,11,37], Geriatric Depression Scale (GDS) [18,36,38,39], Hamilton Depression Scale (HRSD) [35], and the Beck Depression Inventory (BDI) [34].…”
