Determination of adequate pelvic lymph node dissection range for Japanese males undergoing radical prostatectomy

Abstract: Abstract. The present study aimed to determine the adequate pelvic lymph node dissection (PLND) range for Japanese males undergoing radical prostatectomy. A total of 467 Japanese patients who underwent antegrade radical prostatectomy at the National Kyushu Cancer Center (Fukuoka, Japan) were retrospectively reviewed. The patients were divided into two groups according to the PLND extent: The standard (obturator + internal iliac nodes) group and the expanded (standard + additional nodes) group, which accounted … Show more

“…The presence or absence of PLND, the extent of PLND, and nerve-sparing approach were determined by the surgeon’s preference or the policy of each institution. The extent of PLND was categorized as follows: limited (including the obturator fossa only); and extended (performed up to the crossing of the common iliac vessel-ureters, with or without the presacral lymph nodes) [ 16 , 17 ].…”

“…Table 2 and 6 report on studies comparing RT with or without ADT and ADT duration [13][14][15][16][17][18]23,26,27,29,30,32,[65][66][67][68][69]. Table 3 and 7 compare different RT modalities (including BT) [11,12,[19][20][21][22]24,25,28,33,34, and table 4 and 8 compare different RP modalities [16,31,[93][94][95][96][97][98]. A separate table was created for studies reporting on RT with doses below 70Gy (supplementary table 9-10).…”

“…Eight studies compared different RP modalities [16,31,[93][94][95][96][97][98], of which only one was a RCT [31], comparing salvage with adjuvant RT after RP in a high-risk PCa population.…”

“…These results may improve the level of evidence (12). In the previous study, we reviewed all cases, including a low-risk group, and concluded that standard PLND is appropriate at radical prostatectomy (23). In this study, we reviewed the cases in the intermediate-and high-risk groups, and excluded cases in the low-risk group.…”
