“…In the past decade, many efforts have been made to identify drugs that can ameliorate CF activation after MI, such as renin angiotensin aldosterone system (RAAS) inhibitors, inflammation modulators, β-blockers, endothelin antagonists, as well as transforming growth factor beta (TGF-β) inhibitors. Nevertheless, clinical translation of these antifibrotic treatments is mostly hindered by the elusive mechanism of fibrosis, and more importantly, side effects associated with systemic toxicity due to a lack of heart-specific targeting . For example, although it is well received that TGF-β plays a central role in CF activation, systemic administration of pirfenidone, a Food and Drug Administration-approved TGF-β inhibitor, may lead to serious adverse events including gastrointestinal adverse reactions, cardiac valve defects, and systemic photosensitization. , It should be noted that none of the above approaches has led, so far, to an actual translational phase, remaining at the “proof-of-principle” stage. Therefore, there is an urgent need to find potential treatments to target CFs directly with improved safety and efficacy.…”
