Development and Effects of a Transition Nursing Program for Patients and Family Caregivers at a Neurological ICU in Korea

Abstract: The purpose of this study was to develop a transition nursing program for patients and family caregivers in neurological intensive care units (ICUs) and to evaluate the effects of the program. A literature review, focus group interviews, analysis of medical records, confirmation of validity, and clinical applicability were used to develop the program, which was tested using a pre- and post-test quasi-experimental design. Participants were recruited in 2011. The experimental group (46 patients and their family … Show more

“…26,30 Several studies found that transition anxiety scores of relatives of ICU patients decreased after implementing a liaison nurse service. 10,31,32 The patient-and family-centred care (PFCC) policy causes a culture shift in hospitals. 33 This study shows the needs for PFCC interventions and the need to practice this policy not only within the walls of the ICU but also through the whole hospital system.…”

“…However, the study carried out by Janet, et al, indicated that providing information and writing a diary of patients, could decrease the level of family anxiety in the intervention group from 45.18 to 40.42 which was quite similar to the control group that showed a decrease in anxiety level from 46.52 to 39.53, revealing that providing information could not reduce their anxiety level (22). Another study by Yan et al showed that training families with neurologically hospitalized patients in ICU through focus groups and interviews, could reduce their anxiety to some extent, but this reduction was not statistically significant (17). One of the reasons for insignificance of anxiety reduction in this study can be due to the existence of less anxiety in the control group in comparison to the intervention group before the study.…”

[16,17]. Experiencing critical illness and requiring admission to an ICU can be extremely frightening and traumatic, causing distress and disorder in the life of the individual involved and their relatives [18].…”

“…Inadequate continuity of care: Recently, professional caregivers in health-related fields have become aware of major issues in the quality of people's lives and continuity of care post discharge from intensive care unit [16]. the failure to have continuity of care during multiple transitions can lead to medical errors, miscommunication, and care that conflicts with the wishes of the patient and family.…”
