Development and validation of an observational measure of symptoms of Reactive Attachment Disorder

Abstract: Reactive Attachment Disorder (RAD) is presumed to be a consequence of social neglect and deprivation of the kind particularly associated with institutional care. Despite its clinical relevance there is a lack of assessment tools for RAD based on the direct observation of child-caregiver interaction. Here we describe the development and validation of such a tool for use with preschool children, the Rating of Inhibited Attachment Disordered Behavior (RInAB). The RInAB is composed of 17 ratings grouped in three s… Show more

“…Finding that both emotional and behavioral problems may co-occur with RAD and be associated with RAD symptom load in adolescence is concordant with previous findings among school-aged children and early adolescents [ 26 , 41 43 ]. However, this result contradicts that of studies among pre-schoolers, which report that RAD is associated with more emotional problems and not with more behavioral problems [ 39 , 40 , 75 ]. Although there is a possibility of type-II errors where observed differences are not statistically significant due to small sample sizes, the above studies of pre-schoolers used dimensional measures of psychopathology, thereby eliminating potential type-II errors due to reduced power by categorical measures.…”

“…Structured Observation Instruments. Two structured observational instruments are based on the administration of the Strange Situation Procedure (SSP; Ainsworth & Bell, 1970): The Attachment Formation Rating Scale (Zeanah et al, 2005) and the Rating for Inhibited Attachment Behavior (Corval, Belsky, Baptista, Mesquita, & Soares, 2018) for evaluating RAD symptoms. The Rating for Infant-Stranger Engagement (Lyons-Ruth et al, 2009) assesses disinhibited behavior, again during the SSP.…”

“…In this special issue, two observational approaches for the assessment of inhibited RAD for different age groups are presented. The first coding system for infancy and early childhood, the RInAB developed by Corval, Belsky, Baptista, Mesquita, and Soares (2018), assesses three dimensions of inhibited RAD derived from attachment theory. The authors clearly support the assumed internal structure of their assessment approach in a psychometric analysis and examine core attachment related aspects of validity by exploring associations with caregiver sensitivity in institutions in Portugal.…”

“…Indeed, not all children growing up in deprived environments develop inhibited RAD or other forms of psychopathology and insecure attachment patterns beside RAD also are is prominent (Lionetti, Pastore, & Barone, 2015). An attachment figure within an institution or high-quality foster care can reduce inhibited RAD and psychopathology in childhood (Corval et al, 2018;Zeanah et al, 2017). Moreover, genetic polymorphisms (e.g.…”
