Development of unstable hips after treatment with the Tübingen splint: mid-term follow-up of 83 hip joints

Abstract: Our mid-term data show on the one hand a good development of unstable hips after successful treatment with the Tübingen splint. On the other hand despite successful therapy and normal ultrasound findings at the end of treatment further imaging by X-ray are mandatory to close follow-up and to detect those which might need surgical correction of residual dysplasia.

“…Following screening on the pre-specified criteria, including full text review of retained abstracts, eight articles in English and one in German were included in our analysis. 13 , 14 , 19 - 25 Two of these articles, by Kubo et al 13 , 20 used the same study group with outcomes measured at the mid-term and end-point of the period of follow-up. The flow diagram of study selection is shown in Fig.…”

“…In particular, the incidence of residual dysplasia at the six- and 12-month follow-up was 11.7% and 11.8%, respectively. To reduce the incidence of complications, Pavone et al 21 suggested a close follow-up by US, while Kubo et al 20 suggested regular radiographic assessment of acetabular development, as per the Tönnis classification, until skeletal maturity. Ashoor et al 37 evaluated the outcomes across six studies using the THFS (713 children; 1001 hips), reporting a pooled failure rate of 7.79% (0% to 25%), AVN rate of 0.5% (0% to 2.4%) and residual dysplasia rate of 6.7%.…”

“…Pavlik harness or Tübingen Splint and recommend further radiograph controls as well. 25,[36][37][38][39] However, so far, there is no standard algorithm for control examinations after successfully treated DDH. 6 Some authors also recommend other control examinations such as ultrasound and CT. 40,41 Our examinations show a total of 5.4% AVN after the second radiograph control.…”

“…These results foster the recommendation of regular radiological follow-up examinations in numerous other studies. 21 22 Here, an improvement of unstable hips after treatment with the Tübingen splint up to the age of 48 months was seen. Currently, it can be assumed that the therapy of ultrasound unstable hips with the Tübingen splint is a good treatment option when started early.…”
