Diabetes screen during tuberculosis contact investigations highlights opportunity for new diabetes diagnosis and reveals metabolic differences between ethnic groups

Abstract: Type 2 diabetes (T2D) is a prevalent risk factor for tuberculosis (TB), but most studies on TB-T2D have focused on TB patients, been limited to one community, and shown a variable impact of T2D on TB risk or treatment outcomes. We conducted a cross-sectional assessment of sociodemographic and metabolic factors in adult TB contacts with T2D (versus no T2D), from the Texas-Mexico border to study Hispanics, and in Cape Town to study South African Coloured ethnicities. The prevalence of T2D was 30.2% in Texas-Mexi… Show more

“…These findings are in line with the immune response alterations noted in humans and in animal models of TB-DM [18,39,40,44,45]. However, the additional contribution to TB risk conferred by autoimmune or metabolic abnormalities in type 1 or type 2 DM, respectively, cannot be ruled out [43,46].…”

“…However, as indicated in this older literature, the cause of death may not necessarily be TB, but rather, DM complications such as cardiovascular diseases [17,46,49]. Today there is also evidence for other adverse outcomes in TB-DM patients (versus TB-no DM), such as delayed clearance of M. tuberculosis during the course of treatment based on AFB smears or cultures, but these protocols were not established in the early 20 th century [50].…”

“…Third, it has been suggested that the higher death in TB-DM patients is not due to TB, but to DM complications such as cardiovascular diseases [49, 50]. Accordingly, in a recent principal component analysis we found that cardiovascular risk factors (low density cholesterol, triglycerides and central obesity) were correlated with factors defining DM (haemoglobin A1c (HbA1c) and insulin resistance) in participants from South Africa, but not in Mexican Hispanics [51]. Finally, in our study a proportion of the TB patients (4.7%) were not followed when they defaulted treatment, and we were not able to assess other poor outcomes such as TB recurrences and relapses, which are reportedly associated with DM [13, 52].…”

“…Such animal models mimic clinical observations from individuals with TB and diabetes co-morbidity and are particularly useful to study immune responses at the site of infection, the lung, which is difficult to achieve in patients. Given the high global prevalence rates of pre-diabetes in TB household contacts from both low and high TB burden countries -with 23% and 25% in South Africa and South Texas, respectively (Restrepo et al, 2018) -it is imperative to expand TB and diabetes association studies to include pre-diabetes. No animal models of pre-diabetes and TB have been described to date.…”
