Diagnosing phyllodes tumours of the breast: how successful are our current preoperative assessment modalities?

Abstract: In the preoperative diagnosis of phyllodes tumours of the breast, ultrasound was a more discriminating imaging modality compared to mammography, and core biopsy demonstrated a superior accuracy of diagnosis over fine needle biopsy. A significant increase in lesion size over a short timeframe should also alert to the possibility of a phyllodes tumour.

“…However, PTs disturb the balance, with overgrowth of stroma, resulting in a leaf-shaped appearance. According to the World Health Organization, PTs are classified as benign, borderline, and malignant based on five factors: the cellular pleomorphism, degree of mitosis, overgrowth of the stroma, appearance of the margin, and distribution of stoma [ 4 ]. Local recurrence can occur in all subtypes of PTs.…”

“…This is in agreement with a previous study which found that mean lesion size increased when comparing benign, borderline and malignant phyllodes tumours. (18) However, this study did not include any fibroadenomas.…”

“…Previous studies have compared the imaging features of fibroadenomas and phyllodes tumours (1,16,17) but none has compared the imaging features of those lesions requiring excision with a margin and those which do not. (18) In this study, we aimed to identify ultrasound and mammographic features associated with borderline and malignant phyllodes versus benign phyllodes and fibroadenomas to aid surgical planning. Attention was paid to features previously described to be suggestive of a phyllodes tumour.…”

“…Diagnosed as malignant phyllodes tumor. When the phyllodes tumor does not have all the malignant histological characteristics of malignant phyllodes tumors, then it is diagnosed as a benign phyllodes tumor ( 5 , 6 ).…”
