Diagnosing Symptomatic HIV-Associated Neurocognitive Disorders: Self-Report Versus Performance-Based Assessment of Everyday Functioning

Abstract: Background HIV-associated neurocognitive disorders (HAND) include both “asymptomatic” and “symptomatic” neurocognitive impairment. Asymptomatic diagnoses indicate HAND without demonstrable functional impairment. The present study compares three approaches to assess functional decline: 1) self-report measures only (SR); 2) performance-based measures only (PB); and 3) combining SR and PB measures (SR+PB). Methods We assessed 674 HIV-infected research volunteers with a comprehensive neurocognitive battery; 233 … Show more

“…Women had an gradual decline in both health and functionality at age 50 years, while the decline for men became more noticeable at 60 years or older. (16)(17)(18) Our findings highlight the importance of using a hierarchical scale of functional ability that considers assessment of gradual loss of function with aging, mainly in aging with AIDS. Although functional ability was evaluated with instruments widely used and recognized in the gerontology literature, most studies of aging and functionality do not include older people with HIV/AIDS.…”

“…Third, we employed self-report of IADL instead of performance-based assessment. Although self-reporting facilitated the inclusion into the current battery of assessments, reliance of self-report may be biased by depressive symptoms 40. Fourth, aNIHSS and GDS scores might not completely discriminate the effects or presence of any motor impairment or depression.…”

“…[4] One of the key questions is whether the diagnosis of ANI has any relevance in the course of HIV-infection: recent data suggest that patients with ANI may progress to MND and that they have a significant impairment in performance-based tests (potentially affecting adherence to medications). [25,26] The uncertainty in this area is enhanced by the diagnostic criteria that, to some extent, may overestimate the prevalence of asymptomatic and mild forms on neurocognitive impairment.…”
