Diagnosis and Management of Hydatid Disease of the Liver

Abstract: Since 1967, 40 patients with hydatid disease of the liver have been treated at our hospital. Diagnosis was made using clinical criteria, serology, skin tests, and imaging techniques. Thirty-five patients were operated upon. In 18 patients the cyst was uncomplicated (Group I), and in 17 the cyst was infected or communicated with the biliary tract (Group II). Three forms of surgical treatment were used: A) cyst evacuation, scolicidal irrigation, and primary cyst closure, B) evacuation, irrigation, and external d… Show more

“…Cyst fluid spillage can occur during surgery resulting in anaphylaxis and/or secondary echinococcosis (2%-25% of cases). 10,[33][34][35] Two benzimidazoles (mebendazole and albendazole) and praziquantel, an isoquinolone derivative, have scolicidal activity and have been used in over 1000 well-documented patients with cystic hydatid disease. Mebendazole is poorly absorbed, and scolicidal drug levels are achieved with a daily dose of 40-50 mg/kg administered orally in divided doses.…”

“…Balik et al 11 also found that about one-fifth (22.69%) of their patients had fever and 5.92% patients had jaundice. Jacob et al 12 also had eight patients (20%) with jaundice and nine patients (22.5%) had fever. The majority of the hydatid cysts in man are seen in the liver (approximately 75%) because it is the first filtering system for all the ingested ova, which enter the portal system.…”

“…10 Our study established that pain and lump in the abdomen were the common presenting features of CE. According to Balik et al 11 and Jacob et al, 12 pain in abdomen has been the most common presenting symptom (74.01% and 85%, respectively). This was followed by the presentation of lump in abdomen, which was seen in 54.93% cases by Balik et al 11 and 57.5% cases by Jacob et al 12 Contrary to this, Mehta et al 7 concluded that lump in abdomen was the most common presentation in hydatid disease (54.16%).…”

“…Biliary fistulas are the most common morbidity following hydatid liver surgery with an incidence of 13%-37% (20,(21)(22)(23)(24)(25). It is defined as any drain output consistent with a biliary appearance with a high bilirubin count.…”
