Diagnosis and management of Spigelian hernia: A review of literature and our experience

Abstract: Spigelian hernia occurs through slit like defect in the anterior abdominal wall adjacent to the semilunar line. Most of spigelian hernias occur in the lower abdomen where the posterior sheath is deficient. The hernia ring is a well-defined defect in the transverses aponeurosis. The hernial sac, surrounded by extraperitoneal fatty tissue, is often interparietal passing through the transversus and the internal oblique aponeuroses and then spreading out beneath the intact aponeurosis of the external oblique. “…A Spigelian hernia, which was first described by Klinkosch in 1764, accounts for about 0.1% of all abdominal hernias [5]. This is a defect that occurs in the Spigelian facia located between the transversus abdominis aponeurosis lateral to the rectus muscle at the level of the arcuate line [1]. This type of hernia is most commonly seen in females with a mean age of over 60 years old [2].…”

“…The Spigelian fascia is the part of the transversus abdominis aponeurosis lateral to the rectus muscle, often at the level of the arcuate line, where the fascia is widest and weakest [1]. Hernias through a defect in this fascia account for 0.1% of all abdominal wall hernias [2].…”

“…Some authors have suggested that almost 50% of patients with spigelian hernias have had previous abdominal operations, either open or laparoscopic. Other risk factors include sex (male : female—1 : 1.18) [13], with most diagnoses being made between the fourth and seventh decades of life [14]. …”

“…The extraperitoneal placement of mesh is better than transperitoneal technique as there isno chances of intestinal obstruction and fistulization of bowel [16] [17]. As compared to transabdominal extraperitoneal approach, the TEP approach eliminates the complications related to violation of peritoneal layer to reach the preperitoneal space.…”
