Diagnosis of Eosinophilic Enteritis With Video Capsule Endoscopy and Double Balloon Enteroscopy With Favorable Response to Corticosteroids

Abstract: Eosinophilic enteritis is a rare disease with nonspecific symptoms, often representing a diagnostic challenge. Video capsule endoscopy (VCE) has enabled examination of the full small bowel. However, capsule retention is an unfortunate complication. We present the case of a female patient admitted for abdominal pain. Appendectomy without resolution of symptoms was performed. A normal computed tomography and magnetic resonance imaging were obtained. The diagnosis was made by VCE and double balloon enteroscopy wi… Show more

“…Interestingly, many case reports and case series have described diagnosis by SBCE of significant pathologies in patients with chronic abdominal pain (e. g., Meckel's diverticulum [265], eosinophilic enteritis [266], and SBTs [220]). However, the available evidence highlights that the probability of detecting significant findings at SBCE is very low (below 20 %) when isolated chronic abdominal pain is the indication for SBCE.…”

“…In one of the three cases, the SBCE was stuck in the ulcerated ileal stricture with mucosal fissuring, and partial ileal resection was performed to retrieve it ( 7 ). In the other two cases, the capsule endoscope was retained in circumferential ulcerative stenosis, although the SBCE was spontaneously eliminated without clinical symptoms in one case ( 9 ) and with oral budesonide treatment in the other ( 13 ). The frequency of SBCE retention and such circumferential stenosis in EGE is unclear, so an evaluation of gastrointestinal patency may need to be considered before SBCE in cases of suspicious EGE using a patency capsule test.…”

“…In patients with involvement of the muscular layer, narrowing of the lumen can sometimes be seen on endoscopy (89). Capsule endoscopy can also be used for more detailed examination of mucosal changes in the small bowel in case of mucosal disease (90,91) but should not be used if narrowing of the GI lumen is suspected.…”
