Diagnosis of Human Prion Disease Using Real-Time Quaking-Induced Conversion Testing of Olfactory Mucosa and Cerebrospinal Fluid Samples

Abstract: IMPORTANCE Early and accurate in vivo diagnosis of Creutzfeldt-Jakob disease (CJD) is necessary for quickly distinguishing treatable from untreatable rapidly progressive dementias and for future therapeutic trials. This early diagnosis is becoming possible using the real-time quaking-induced conversion (RT-QuIC) seeding assay, which detects minute amounts of the disease-specific pathologic prion protein in cerebrospinal fluid (CSF) or olfactory mucosa (OM) samples.OBJECTIVE To develop an algorithm for accurat… Show more

“…We consider this a finding indicative of PrP infection because the present case had short disease duration and the olfactory bulb contained postmitotic tissue. In support of this hypothesis, there have been several reports showing that PrP Sc could be detected in the olfactory mucosa . The olfactory bulb is connected to the olfactory mucosa through the olfactory nerve and the latter is in contact with the external environment.…”

The olfactory bulb may be a possible route of PrP infection in sCJD.…”

“…Protein 14‐3‐3 was positive in 6 of 12 tested patients (50%). Real‐time quaking‐induced conversion (RT‐QuIC) test results for GSS were only reported in a few articles and appeared to have a sensitivity up to 75 to 78% …”

“…Such testing can provide intra vitam diagnoses that can be virtually 100% sensitive and specific [2, 34]. We and others have recently described α-synuclein RT-QuIC and closely related assays for the CSF-based early diagnosis of Parkinson disease (PD) and Lewy body dementia (LBD) [11, 18, 41].…”
