Diagnosis of Patent Foramen Ovale: The Combination of Contrast Transcranial Doppler, Contrast Transthoracic Echocardiography, and Contrast Transesophageal Echocardiography

Abstract: Objectives. To access the distinct values of contrast transcranial Doppler (cTCD), contrast transthoracic echocardiography (cTTE), and contrast transesophageal echocardiography (cTEE) in the diagnosis of right-to-left shunt (RLS) due to patent foramen ovale (PFO) and to define the most practical strategy for the diagnosis of PFO. Methods. 102 patients with a high clinical suspicion for PFO had simultaneous cTCD, cTTE, and cTEE performed. The agitated saline mixed with blood was used to detect right-to-left shu… Show more

“…Enrolled patients were screened for PFO using contrast-enhanced transcranial Doppler ultrasound (cTCD). 17 For cTCD test-positive patients, after informing the patient of the condition and following the patient’s consent, transesophageal echocardiography or right heart catheterization was conducted to confirm it. cTCD uses a single-channel dual-depth mode to monitor the middle cerebral artery and chooses the vertebral artery in patients with poor temporal window sound penetration.…”

“…Pre-pneumonectomy is one such example [ 1 ]. Shunt fraction can be quantified over time in patients with a continued oxygen requirement by evaluating the intracardiac shunt directly with serial echocardiography, transcranial Doppler, and right heart catheterization [ 28 31 ].…”

“…Following the directions of the echocardiographers, saline contrast was administered through an antecubital vein by bolus injection while evaluating the four-chamber apical view. The evaluation method was as follows ( 9 , 10 ): the potential appearance of microbubbles, its timing, and its hunt origin were permitted to be intuitively observed. The presence of microbubbles derived from the pulmonary veins was classified as a pulmonary RLS ( Figure 2 ).…”
