Diagnosis of smear-negative pulmonary tuberculosis based on clinical signs in the Republic of Congo

Abstract: BackgroundThe diagnosis of pulmonary tuberculosis (PTB) and smear-negative pulmonary tuberculosis (SNPT) in resource-limited countries is often solely based on clinical signs, chest X-ray radiography and sputum smear microscopy. We investigated currently used methods for the routine diagnosis of SNPT in the Republic of Congo (RoC) among TB suspected patients. The specific case of HIV positive patients was also studied.MethodsA cross-sectional study was conducted at the anti-tuberculosis center (CAT) of Brazzav… Show more

“…Particular attention should be paid to the diagnosis of sputum negative TB. In areas with a higher prevalence of TB and HIV, the clinical signs and inexpensive tests, such as direct microscopy, tuberculin skin test and chest radiography, are of great importance in the diagnosis of PTB 29,30 . Anorexia, asthenia and a less persistent cough are good predictors of SNPT.…”

“…These symptoms thus deserve to be recommended as indicators of the SNPT diagnosis within an ATB dispensary. This strategy can help reduce morbidity and mortality associated with a late SNPT diagnosis 30 . Sputumnegative TB was present in 40% of those affected in Northern Kosovo, which is consistent with the results of other studies 5,15,30,31 , while the atypical radiographic changes occurred in 23% of the affected.…”

“…Diagnosing a neglected disease like melioidosis with a similar clinical presentation like TB in this milieu is challenging for the treating physicians. Patients with a past history of TB are usually diagnosed as recurrence based on suggestive clinical presentations, indicative radiological findings and other non-confirmatory laboratory parameters (ADA profile, high ESR), rather by laboratory confirmation [15,16] and treated with anti-tubercular antibiotics. This may be considered as one of the major causes of misdiagnosis of melioidosis, as it is reflected in our observation of seven cases of melioidosis, who were suspected to have recurrent TB at primary health care settings.…”

“…Posebnu pažnju treba obratiti u dijagnozi sputum negativne tuberkuloze. U područjima sa višom prevalencom TBC i HIV, klinički znaci i jeftina ispitivanja, kao što su direktna mikroskopija, tuberkulinski test i radiografija pluća su od velikog značaja u dijagnostici TBC [17]. Anoreksija, astenija i manje uporni kašalj su dobri prediktori SNTBC.…”
