Diagnostic accuracy of a 2D left ventricle hypertrophy score for familial hypertrophic cardiomyopathy

Abstract: The LVH score has a higher diagnostic value for HCM than the conventional criterion of MWT, particularly in young adults. This echographic parameter may be proposed as an alternative diagnostic criterion for familial screening.

“…Moreover, mutations of sarcomeric protein genes account for only 60% of HCM cases 49 50. Additionally, we used a myocardial wall thickness cut-off of 15 mm47 rather the conventional value of 13 mm,51 which is less accurate than the 2D left ventricular hypertrophy score. Radial, transverse or circumferential left atrial strain were not measured because left atrial wall is too thin 30.…”

“…For example, in studying hypertrophic cardiomyopathy, one may be interested in maximal wall thickness (MWT), interventricular septum thickness (IVS) or, as proposed by Spirito and Marron (1990), a score combining several measurements of left ventricular hypertrophy (LVH score). All these variables must be adjusted for appropriate covariates, such as age, sex and body surface area (Forissier et al 2005). Success in Wnding genetic factors may depend on the choice of variable and on the inclusion of appropriate covariates in the model.…”

“…In our patient, echocardiography was performed at admission and detected a severe LVOT obstruction, associated with septal bulge and systolic anterior motion of the mitral valve and mitral regurgitation. This morphological pattern of the interventricular septum is mostly presented in older patients and seems to be an important factor of LVOT obstruction in TakoTsubo syndrome, mimicking a pattern of obstructive hypertrophic cardiomyopathy [11].…”
