Diagnostic accuracy of magnetic resonance enterography and small bowel ultrasound for the extent and activity of newly diagnosed and relapsed Crohn's disease (METRIC): a multicentre trial

Abstract: SummaryBackgroundMagnetic resonance enterography (MRE) and ultrasound are used to image Crohn's disease, but their comparative accuracy for assessing disease extent and activity is not known with certainty. Therefore, we did a multicentre trial to address this issue.MethodsWe recruited patients from eight UK hospitals. Eligible patients were 16 years or older, with newly diagnosed Crohn's disease or with established disease and suspected relapse. Consecutive patients had MRE and ultrasound in addition to stand… Show more

“…Meta-analyses suggest that small bowel ultrasound (SBUS) achieves a high sensitivity for the presence and extent of small bowel Crohn's disease (CD), recently confirmed in a prospective multicentre trial setting by the METRIC trial [1][2][3][4]. SBUS has several advantages over Magnetic resonance enterography (MRE).…”

“…Our findings support its wider adoption in this age group. Further dedicated prospective comparative studies of SICUS and MRE, such as the United Kingdom‐based MR Enterography or ulTrasound In Crohn's disease (METRIC) trial in adults, are now needed in children …”

“…Crohn’s disease and ulcerative colitis are the two principal subtypes of IBD and involve chronic immunological response in the gastrointestinal mucosa. Ileocolonoscopy with biopsies is still considered the gold standard for initial diagnosis, disease activity monitoring and assessment of therapeutic response( 3 ). Unfortunately, this procedure is costly, causes significant patient discomfort, can lead to intestinal perforation and is limited to the evaluation of the lower part of the gastrointestinal mucosa (rectum, colon and terminal ileum)( 4 ).…”
