Diagnostic accuracy of two endocervical sampling method: randomized controlled trial

Abstract: Endocervical brushing was proved to be as accurate with respect to diagnostic yield as ECC but less painful, evaluating the endocervical canal. It can be an alternative for evaluation of the cervical canal pathology in patients with low grade cytologic abnormalities but devoid of the misinterpretation of stromal invasion; ECC should be preferred in patients with a suspicion of invasive disease.

“…It is interesting to note that in our study we conclude the exact opposite: one of the reasons for the better adequacy in our specimens of the ECB groups is the tendency of the physicians to apply an increased pressure during the procedure due to the perception of decreased pain in this group of patients. Goksedef et al [ 7 ] is the final study that found a benefit in the use of ECC compared to ECB. They prospectively randomized 208 patients to either ECB or ECC.…”

“…One of the fundamental steps in the diagnosis of dysplasia and cervical cancer is colposcopy and the evaluation of the endocervical canal. The latter is usually done by endocervical curettage, but due to its high rate of false negative results and important patient discomfort, efforts have been made to find alternative techniques [ 4 7 ].…”

“…Endocervical brushing has better sensitivity than curettage with similar specificity, better tolerance, and fewer insufficient samples, although grading may be more difficult because stroma is rarely sampled with brushing. 15,16 Either is acceptable for endocervical sampling. In 2006, working groups assessing management of cytology reported as atypical squamous cells of undetermined significance (ASC-US) and low-grade squamous intraepithelial lesions (LSIL) defined indications for endocervical sampling, guidance that should be valid for women with cytology results of Box 1.…”

“…In the study thus identified, Goksedef et al 11 to present low prevalence. Moreover, those authors used a different brushing technique for sample collection: the material obtained through brushing was fixed in formaldehyde and processed for embedding in paraffin blocks, followed by histological analysis (rather than using conventional cytological analysis).…”
