Diagnostic Accuracy, Prescription Behavior, and Watchful Waiting Efficacy for Pediatric Acute Otitis Media

Abstract: Current guidelines recommend “watchful waiting” (WW) as an alternative to immediate antibiotic treatment. Continued high rates of antibiotic use suggest that WW may be underutilized. We conducted a retrospective chart review of 474 pediatric acute otitis media (AOM) cases at a clinic in central Pennsylvania. We assessed physical examination findings, diagnostic behavior, WW utilization, prescription writing, and filling in cases of pediatric AOM to evaluate the underutilization of WW. We evaluate diagnostic co… Show more

“…20 In this study, more than 80% of OM diagnoses received antibiotic prescriptions, and at UCC and PMD, almost 40% were for non–first-line agents. Recent analyses find that WW is appropriate for most OM cases by guideline criteria, but is rarely used 13,21 ; WW has also been shown to be safe and cost-effective at a programmatic/societal level. 13 In addition to encouraging delayed prescribing for less severe OM cases, avoiding the regular use of azithromycin and oral third-generation cephalosporins (~29% at PMD and UCC) stands to be reinforced, as these non–first-line agents may be not only excessively broad-spectrum, but also less effective.…”

“…Despite the frequency of such encounters, the medical literature suggests that the diagnostic accuracy for such pathology is only around 46-56%. In alignment with the bias toward over-diagnosis of ear disease, it is currently estimated that between 25-50% of all antibiotics prescribed for ear disease are not indicated (1)(2)(3). Beyond risking unnecessary medical complications and the downstream unintended consequence of potential antibiotic resistance, over-diagnosis of ear disease adds an estimated $59 million in unnecessary healthcare spending in the US per annum (4).…”
