Diagnostic and therapeutic approach to infectious diseases in solid organ transplant recipients

Abstract: Prognosis of solid organ transplant (SOT) recipients has improved, mainly because of better prevention of rejection by immunosuppressive therapies. However, SOT recipients are highly susceptible to conventional and opportunistic infections, which represent a major cause of morbidity, graft dysfunction and mortality. Methods: Narrative review. Results: We cover the current epidemiology and main aspects of infections in SOT recipients including risk factors such as postoperative risks and specific risks for diff… Show more

“…Preventing these infections will be a major challenge. Indeed, many of these infections involve the surgical site and the allograft itself, and are favored by surgical complications including anastomotic leaks and strictures, undrained collections, and indwelling catheters [ 3 , 33 , 34 ]. Major efforts to reduce these risk factors, including improved surgical techniques, adequate source control, and early removal of indwelling catheters and prosthetic material will be essential.…”

“…Patients after solid organ transplantation are at increased risk for nosocomial infections due to multidrug-resistant bacteria [15] and particularly to VRE bacteraemia [10,16,17]. Mortality rates up to 46% in the context of VRE bacteraemia have been described in these patients [10].…”

“…Despite advances in liver transplantation, morbidity and mortality due to infectious complications remain the biggest problem [114,115]. In many centers, infection is the leading cause of death after liver transplantation, particularly after the first year [116].…”
