Diagnostic and Therapeutic Joint Injections

Abstract: Joint injection is a useful tool in the diagnosis of intra-articular pathology that may improve diagnostic performance of computed tomography (CT) and magnetic resonance (MR) imaging. Historically, conventional arthrography under fluoroscopy was the first method to be used to image indirectly the intra-articular soft tissues, but with the advent of CT, CT arthrography offered better soft tissue depiction. The development of conventional MR allowed even better visualization of soft tissues, and in the early 199… Show more

“…RF treatment has been used for several painful conditions such as trigeminal neuralgia, cancer pain, and spinal pain. In order to destroy nerves or disrupt the transmission of pain signals, originally by means of producing heat lesions, RF current is applied to the trigeminal ganglion, the spinothalamic tracts of the spinal cord, the medial braches of posterior rami and the dorsal root ganglion [4]. Therefore, it is essential to place the electrode tip as close as possible to the target nerve: there are several reasons for the difficulty in localising the target nerve, for example the variability of the anatomical course of each target nerve and the sensory nature of target nerves.…”

“…Conservative approaches include physical therapy, analgesic drugs, intra-articular injection of steroids, and visco-supplementation [3,4]. Pharmacological therapy is often of limited benefit for OA pain, but non-steroidal anti-inflammatory drugs are associated with serious side effects, such as bleeding and gastrointestinal ulcers.…”

“…Intra-articular contrast medium injection for CT and MR arthrography is performed under fluoroscopy control and sterile conditions by means of anterior or medial access while avoiding the dorsalis pedis artery by means of a 20 -22 gauge needle [7,23]. For the arthrography, a test infusion of local anesthetic (e. g., lidocaine 2 %) as well as iodine-based contrast agent is used to confirm the intra-articular needle position.…”

“…Eklem içi ligamanların rüptürü , osteokondral defektler, impingement, adheziv kapsülit ve loose body tanılarında artrografik yöntemler kullanılmaktadır (4). BT ve MR artrografi gibi tanısal yöntemler için eklem mesafesine kontrast madde enjeksiyonu yapılabilir (5)(6)(7). İntraartiküler enjeksiyon için farklı metodlar bugüne kadar kullanılmıştır.…”
