Diagnostic evaluation of sentinel lymph node biopsy in early head and neck squamous cell carcinoma: A meta‐analysis

Abstract: The SLNB procedure has shown a high sensitivity rate, but the pooled sensitivity and false-negative rate were worse in SLNB alone trials than in SLNB validation trials.

“…8,9 Although SLNB might reduce part of these issues, 10 it is a time-consuming and technically challenging procedure, which still does not prevent an open surgical approach. 1,2 Elective neck dissection is currently regarded as the treatment of choice for HNSCC patients with a cN0 status.…”

“…Sentinel lymph node biopsy (SLNB) is less invasive than up-front neck dissection and holds promises in HNSCC. 10 However, SLNB is still an invasive procedure. In addition, this technique is time-consuming, technically challenging, and requires a close collaboration between different medical specialties, making its use only possible in dedicated centers.…”

“…Furthermore, most previous individual studies contained too small of a sample size to yield a valid conclusion. In addition, previous meta-analyses mainly focused on head and neck cancer or oral and oropharyngeal carcinoma [1316]. However, combining different subset of head and neck cancer with differing clinical characteristics and metastasis patterns, can lead to heterogeneous results for SLNB.…”

“…The pooled false-negative value calculated by a meta-analysis of SLN biopsy studies for head and neck SCC was 13.7%. 76 On the basis of the limited experience with SLN biopsies of SCC of the periocular region, the positivity rate is 12.5% (1/8) 22 to 14% (1/7) 25 with no reports of false-negative biopsies as of yet. One article describes positive SLN biopsy findings in all 5 patients with periocular invasive SCC in their series.…”
