Diagnostic performance of a computer-assisted diagnosis system for bone scintigraphy of newly developed skeletal metastasis in prostate cancer patients: search for low-sensitivity subgroups

Abstract: PurposeThe computer-assisted diagnostic system for bone scintigraphy (BS) BONENAVI is used to evaluate skeletal metastasis. We investigated its diagnostic performance in prostate cancer patients with and without skeletal metastasis and searched for the problems.MethodsAn artificial neural network (ANN) value was calculated in 226 prostate cancer patients (124 with skeletal metastasis and 101 without) using BS. Receiver operating characteristic curve analysis was performed and the sensitivity and specificity de… Show more

“…We have investigated that BONENAVI ® often resulted in false-positive or false-negative in patients with small tumor burden (e.g., EOD = 1 and low activity) [ 24 ]. Others have also reported that BSI was strongly influenced by tumor location, volume, and scan uptake time in a simulation study [ 25 ] and clinical setting [ 26 ].…”

“… 25 Internal fixation may be performed if the expected survival is >6 weeks, the patient’s general condition is suitable for surgery, mobilization is expected after internal fixation, the structure and quality of the bone both proximal and distal to the focal bone lesion can support the internal fixation, and/or pain persists despite ≥4 weeks of a sufficient dose of radiotherapy. 26 …”

“…Regarding detection of PCa metastases, Koizumi et al reported the ability of an ANN algorithm, to identify skeletal metastases with an accuracy of 80% on bone scintigraphy. However, regarding the detection of non-skeletal metastasis, the performance of the model decreased to 22%, [39]. After treatment, metastatic lesions stand sclerotic and it can be difficult to identify recurrence.…”
