Diagnostic role of Xpert-MTB RIF assay in osteoarticular tuberculosis: A retrospective study

Abstract: BACKGROUND Osteoarticular tuberculosis (OATB) is a severe form of extrapulmonary tuberculosis (TB), which causes notable morbidity and warrants a high index of suspicion for prompt management. The diagnosis of OATB poses a challenge, because of the difficulty of collecting the samples and, secondarily, the paucibacillary nature of lesion, which gives poor sensitivity and reproducibility, with long turnaround time of routine/conventional laboratory tests and the requirement for invasive procedures … Show more

“…Cartridge based nucleic acid amplification tests (CBNAAT) or the GeneXpert assay and TRUNAT (a chip based micro-PCR platform) assays are really helpful for the initial diagnosis of Rifampicin resistance, can also act as a surrogate marker for MDR TB. These tests are rapid, simpler and have high sensitivity and specificity [ 12 ]. In our survey, 88.8% of practitioners use GeneXpert for initial diagnosis.…”

“…Finally, even in endemic countries like India, the government has come up with a recommendation to perform a biopsy whenever possible 12 . Molecular testing, such as the GenExpert (Cepheid), has made the diagnosis simple 24,25 . The pus can be obtained from more accessible sites, such as a psoas abscess in our case, which has become a routine for every patient with bright psoas on MRI.…”

“…Rapid molecular techniques such as GeneXpert MTB/ Rif assay have made the detection of MTB in STB easy, fast, and simple [ 11 ]. However, the main diagnostic dilemma in STB remains is sample collection from deeper sites, and paucibacillary nature of disease [ 12 ]. Chemotherapy is treatment for STB and surgery is indicated in selective cases [ 4 ] Nevertheless, recovery after rapid-onset paraplegia remains guarded [ 13 ].…”
