Current diagnostic techniques are inefficient in distinguishing latent and low-risk forms of prostate cancer from high-risk forms. The present study is focused on determination of putative tumor markers of aggressive high-grade forms of prostate cancer. Potential markers were determined in blood sera of 133 patients (82 cases and 51 controls) and in cell lines (Gleason score 9-derived 22Rv1 and normal tissue derived PNT1A) on mRNA and protein levels. Alpha-methylacyl-CoA racemase (AMACR), metallothionein classes 1A and 2A (MT1A and MT2A) were determined and compared to prostate specific antigen (PSA) levels. On mRNA level, significantly increased expression of MT2A (2.4-fold), PSA (2.6-fold) and AMACR (8.4-fold) and insignificantly (1.9-fold) elevated MT1A in 22Rv1 compared to non-tumor PNT1A were determined. On protein level, significant enhancement of free PSA and total PSA in tumor cell line was evident. AMACR protein was 1.5-fold elevated in tumor line (below the level of significance). Contrary to mRNA, significantly (p = 0.01) reduced level of MT protein in tumor lines was determined. In the case of serum level, significantly enhanced MT level (4.5-fold) in patients' sera was found. No significant changes were observed in the case of AMACR. These findings indicate possible alternative role of MT to PSA prostate cancer marker. In addition, level of AMACR is distinctly higher in the Gleason score 9 in serum of patients and MT shows a descending trend in relation to Gleason score.

Serum levels of Prostate-Specific Antigen (PSA) are not fully specific for prostate cancer (PCa) diagnosis and several efforts are focused on searching to improve PCa markers through the study of PSA subforms that could be cancer associated. We have previously reported by 2DE a decrease in the sialic acid content of PSA from PCa compared to benign prostatic hyperplasia patients based on the different proportion of the PSA spots. However, faster and more quantitative techniques, easier to automate than 2DE, are desirable. In this study, we examined the potential of CE for resolving PSA subforms in different samples and compared the results with those obtained by 2DE. We first fractionated by OFFGEL the subforms of PSA from seminal plasma according to their pIs and analyzed each separated fraction by 2DE and CE. We also analyzed PSA and high pI PSA, both from seminal plasma, and PSA from urine of a PCa patient. These samples with different PSA spots proportions by 2DE, due to different posttranslational modifications, also presented different CE profiles. This study shows that CE is a useful and complementary technique to 2DE for analyzing samples with different PSA subforms, which is of high clinical interest.

Aim: Prostate cancer (PCa) is the second most prevalent male cancer worldwide and designated the sixth most frequent male cancer in Arab countries. Although prostate specific antigen (PSA) has become the best and most valuable biomarker for screening of PCa, elevated levels of PSA can reflect the presence of malignant cells but can overlap with benign prostatic diseases. There is a necessity to develop and improve current tools for early detection and diagnosis of PCa. This study was done to evaluate the validation of serum insulin-like growth factor-1 (IGF-1), IGF binding protein-3 (IGFBP-3), chromogranin A (CgA) and combination with PSA in treatment of benign prostatic hyperplasia (BPH) and PCa patients. Methods: The study included 72 patients with PCa, 70 BPH patients and 56 healthy male subjects of matched age. Full history and clinical data were recorded for all subjects. Results: Serum PSA attained sensitivity of 84% at 82% specificity with an accuracy of 83%, although IGF-1, IGFBP-3 and CgA did not recognize PCa patients. Conclusion: Combinations of IGF-1 and IGFBP-3 biomarkers with PSA were effectively differentiated between PCa and control groups as well as improving the overall value of sensitivity, specificity and diagnostic accuracy of PCa to 85% and 86% for IGF-1/PSA and IGFP-3/PSA respectively.
