Diagnostic Value of Combining Bilateral Electrocochleography Results for Unilateral Ménière’s Disease

Abstract: We defined ‘an interaural difference in the summating potential/action potential (SP/AP) ratio from the ipsilateral hearing-impaired side to the contralateral normal-hearing side of ≧0.15’ as a positive result for a novel ‘relative criterion’ for the diagnosis of unilateral Ménière’s disease. A uniform result could be derived only between a positive ECoG result and the side of disease in 78.7% subjects by conventional absolute criterion (SP/AP ≧0.42). By adding the relative criterion, we found that the diagnos… Show more

“…In this study, we performed extratympanic EcochG, of which clinical usefulness in the diagnosis of Ménière's disease was reported in past reports [Chung et al, 2004;Hwang et al, 2008;Pappas et al, 2000;Bonucci and Hyppolito, 2009]. Our results showed that the SP/AP ratio was enlarged in patients with significant endolymphatic hydrops except for 2 patients with a relatively short history of the disease.…”

“…They not only point to the hypothetical initiation of a bilateral form of the disease (which will need a more careful and detailed prospective study) but also to the need to take into account the audiometric data from the contralateral ear as shown recently in electrocochleography studies [Hwang et al, 2008]. Treatment for patients suffering TA must try to avoid any deleterious damage to hearing in the symptomatic ear, as has been our experience with intratympanic gentamicin [Martin and Perez, 2003] that is administered following an injection on demand protocol.…”

“…For instance, the apparent shape of a low frequency sensorineural loss is usually related to Meniere ' s disease (Hwang et al, 2008). Other well-documented shapes include Carhart ' s notch for otosclerosis (Carhart, 1950) and a 4-kHz dip for noise-induced hearing loss (Taylor et al, 1965).…”
