The purpose of this study was to evaluate the relationship between the endolymphatic space image obtained using magnetic resonance imaging (MRI) and the results of electrocochleography. Electrocochleography recordings were obtained from 25 ears of 24 patients, who underwent MRI 1 day after the intratympanic injection of gadolinium diethylenetriamine pentaacetic acid bismethylamide. The average summating potential to action potential (SP/AP) ratio in patients with significant endolymphatic hydrops in the cochlea was 54 ± 17%. However, in some patients who had significant endolymphatic hydrops in the cochlea, the SP/AP ratio was not enlarged. This may imply that elevation of the SP/AP ratio is related to not only the degree of endolymphatic hydrops but also to the persistence of hydrops.

Auditory and Vestibular Assessment of Patients with Ménière’s Disease Who Suffer Tumarkin Attacks

Tumarkin attacks are a feature of Ménière’s disease that generate a significant degree of disability. The surprising nature of these events is the main reason behind their associated morbidity. In this study we set out to evaluate auditory and vestibular function, as well as disability, in a population of patients who suffer Tumarkin attacks. We found that patients who suffer Tumarkin attacks are more disabled and experience severer and more frequent autonomic symptoms and that their hearing level is significantly worse in the asymptomatic ear, especially at low frequencies. Accordingly, we consider that treatment must be carefully planned to be as conservative as possible in terms of hearing and that psychiatric and/or psychological treatment must always be considered as an adjuvant therapy.

The purpose of this study was to design a statistical classification system of audiogram shapes in order to improve and integrate shape recognition across clinical settings. The study included 1633 adult subjects with normal hearing or symmetric sensorineural hearing impairment who underwent pure-tone audiometry between July 2007 and December 2008. K-means cluster analysis was employed to categorize audiometric shapes. Eleven audiogram shapes were identified: rising, flat, peaked 8-kHz dip, 4-kHz dip, 8-kHz dip, mild sloping, severe 8-kHz dip, sloping, abrupt loss, severe sloping, and profound abrupt loss. By using the classification system and nomenclature identified for audiogram shapes as outlined in this study, errors based on personal experiences can be reduced and a consistency can be developed across clinics.
